Pedigree Kit

This kit provides valuable tools to help you organize notifying and gathering information from your relatives. This will help identify family members who need to be screened and could save a life. We hope you will encourage all of your relatives to register in the SADS Pedigree Project!

Please either print or order this kit to hand out or mail to your family. The kit contains a packet for you and mini-packets for each family member.

Instructions:

First, fill out the Family Members to Contact list by entering the names of relatives, living or deceased. Use this list as a guide to help you know who to contact for more information.
Next, complete your short questionnaire and make sure you are informed about SADS conditions. You have to start with yourself first!
Then contact other family members by mail, e-mail or phone to get their information (see sample letter) and either mail them or hand them their mini-packets.
Make sure this data gets back to the SADS Foundation by calling us at 800-STOP-SAD, or faxing the forms to 801-531-0945
