Create a Family Fund
Establishing a fund at the SADS Foundation in honor of someone living and thriving with a SADS condition or ain memory of someone who has passed away is a meaningful way to honor and remember your loved one while making a difference. It is also a simple way for friends and family to make contributions to the ongoing work of the SADS Foundation.
In honor of funds:
Establishing a fund for someone who is living and thriving with Long QT or another SADS condition is a celebration of life. A link to this fund can be placed in e-mails or on your facebook page as you use it for birthday celebrations, anniversaries, or any other occasion.
In memory of funds:
When a loved one has died, we often express our sentiment with a gift in that person’s name as a remembrance. Many times, in lieu of flowers, gifts to a SADS Foundation memorial fund can made a meaningful impact as the SADS Foundation pursues its mission of supporting families and saving lives. If you or someone you know is dealing with the death of someone dear and would like to establish a SADS Foundation Memorial Fund, please understand that the process is as simple as a phone call or email. We will discuss your wishes and handle the administrative services of the fund for you.
How it works:
Use the
Healing Wall Entry Form
to record the name and the story of the person to be honored and to designate if it is a fund in honor of someone (living), or in memory of someone (deceased). Please upload a digital photo with their story.
Check the "Create Fund" option at the bottom of the form, and include a paragraph in the "Fund Description" explaining the importance of the SADS Foundation's work to your family.

with your contact information or call her at 801-272-3023. We also can provide donation envelopes and informational brochures for use at a memorial service.
If appropriate, we will ask for a copy of the obituary or where we may find it.
We provide a thank-you letter to all donors and also provide a list of donors to you so that you may thank them personally yourself.
